Let’s start by breaking down the words Carpal-Tunnel-Syndrome. Carpal is the Latin word for the wrist. The Tunnel is an opening in the wrist created by the eight carpal bones (2 rows of four bones) on the bottom and the transverse ligament on the top. A Syndrome is a group of signs and symptoms that occur together.

The most common signs and symptoms of carpal tunnel syndrome are wrist or hand pain, weakness of grip, numbness and tingling in the arms and hands, night pain (classic), swelling of the hand and fingers.

There are many secondary complaints such as elbow pain, shoulder pain, pain that radiates from the hand to the elbow, from the shoulder to the hand, and even neck pain.

There are eight carpal bones in the wrist that are all interconnected to form the bottom of the carpal tunnel. The transverse ligament forms the top part of the carpal tunnel. The carpal tunnel itself is about the size of the tip of your pinky. Inside the carpal tunnel are three things, the median nerve, the nine tendons of the flexor muscles, and some blood vessels.

Carpal Tunnel Syndrome (CTS) occurs when there is pressure on the median nerve inside the carpal tunnel. This can happen in two ways. The contents of the tunnel can expand. Specifically, the flexor tendons can be overused which leads to inflammation and median nerve compression. Or, the diameter of the tunnel can narrow from misalignment of the carpal bones from trauma or repetitive use or flattening of the transverse ligament from strain or overuse.

Occupations that require repetitive use of the arms and hands are most prone to CTS. Since our clinic is located in the heart of San Francisco, we mostly see office workers. The majority of our patients spend their day at a desk in front of a computer doing repetitive tasks with their hands. Many then go home and do it for some more. With cell phones and tablets, most of us now do it in between work and home on the train, bus, or ferry.

Sports and hobbies that involve repetitive use of the hands will also predispose you to carpal tunnel syndrome. Sports like tennis, golf, and racquetball. Hobbies like bowling and knitting. Musicians are often victims of CTS. Women are more prone to CTS and they also have a smaller carpal tunnel. Pregnant women frequently suffer from carpal tunnel syndrome because the tissues in the wrist swell during pregnancy.

Conditions such as fibromyalgia, headaches, and whiplash are also often times connected with carpal tunnel syndrome.
